EVALUATION QUESTIONNAIRE

Fife Gingerbread would like you to help us to evaluate the services we provide, by filling in a short questionnaire.

There are three types of survey you can fill in:

• If you are an organisation or work with Fife Gingerbread on a professional level
• If you currently use our services and engage with us
• If you are on one of our mailings lists (e-newsletter, newsletter etc) but don't necessarily engage with us regularly

Everyone individual who sends a questionnaire back to us can enter a prize draw to win £50 in High Street Vouchers.

The idea of an evaluation is simple – by knowing what works well (or not so well), we can tell people what we have achieved and plan for the future.

The questionnaire is a chance for you to give your views of Fife Gingerbread. There are no right and wrong answers and we don’t need your name (unless you want to enter the draw). Even if you give your name, your answers will be kept confidential.
